- panels are not collapseable (if i want to edit just patient result of many users then i will have to collapse all of them, it should be closed by default)

- i cant purpose of dropdown for selecting file type on patient form
- Fields validations are missing (Mr Number, Employee #, CNIC, Contact Number)
- All fields are missing error is showing , not specifying field name
- If you donot select hospital first then it will give you all fields are necessary all the time even with all fields filled.
- In Date Of Birth Field select future date . it will give you -1 in age
- Age is 0 all the time
- Save button should be outside of panel
- Cant see paging in listing page
